#🤦♂️#welp#got off the phone w the vet and my dog needs to be on yet another medication#she's developed hypothyroidism which is making her health worse#And her liver enzymes r too high#so we can add that to the list that also includes:#moderate to severe heart murmur#high blood pressure#severe seasonal allergies#extreme IBD#the most severe case of hemorrhagic gastroenteritis the internal med specialist has seen in her 20+ year career#arthritis#permanent spinal injury & condition#.....HOW IS THIS DOG EVEN ALIVE HER BODY IS AS CHAOTIC AND BROKEN AS MINE IS#holy fuck#truly and actually i have spent over $10k on this dog in the last 2 years#currently paying about $300 a month for her medications (which will now go up) plus her 2 specialised diets and 3 supplements#aiyaiyaiiiii#fucking hell#truly i adore this dog but she is draining my bank account like leech#its not her fault obvi its all good#and i can certainly relate as a trash-bodied disabled person as well#but thats. kind of the point#i live off of disability and my mother has no savings left so i cannot afford keeping up w these appts and costs#now they wanna do more ultrasounds and i was like. ma'am. i have zero moneys. i cannae do that#so we compromised for more bloodwork (still pricey af) and if that looks bad then we'll go for the ultrasound#fuckin hell. sudhdhdj#I LOVE THIS DOG WITH ALL HER ISSUES BUT JFC I CANNOT AFFORD THIS#diary#willow

miscommunication aside, it's a huge relief that I won't have to get either braces or invisalign. the cost was bad enough, but even moreso I felt like I'd have to stay here for at least two and a half more years to maintain continuity of care. now, they're saying I'll be done in August. my lease is also up in August! theoretically, there's nothing tying me down anymore. opportunities abound 👀
#there IS the fact that I see several specialists and I need to get testing fairly frequently#and everything is SO smooth since I see people all in the same system#I genuinely hate having to call people and try to arrange all that so much#that the healthcare system I currently have access to is still a significant reason to stick around#also I hate my former GE doctor. turns out I don't actually HAVE cirrhosis yet but the way he spoke to me#you would think I was already dying of it#and that this shouldn't be a surprise. of course I was upset#we shared an elevator later...most awkward ride of my life#anyway my new guy is cool#in fact the liver recipient who spoke a few days ago at my work event#is also his patient!#so its good to know he got thatman a liver when he needed it#cor.txt

yesterday I had an appointment and got weighed. The last few I've had, around three months ago, I was in the 250 range, but this time I was down to 204. Obviously I'm still fat, but I've been exercising more and watching what I eat, but that feels a bit better than before
But the doctor still harped on about my weight and that I need to try harder to lose it
Idk, I feel losing that much should have at least gotten acknowledged...
#Kitty mumbles#If I get another fuckin fatphobe on my vent post telling me to stop stuffing my face I'm gonna actually commit a felony#Kitty vents#Said doctor is my liver specialist so he has access to blood tests and stuff#He's not just telling me to lose weight for no reason#I honestly started feeling good after learning that I lost some weight#But the immediate “lose more” without acknowledging the weight already lost kinda hurt#I'm not tagging main tags here I just wanted to vent#I'm trying to be healthier but it doesn't seem like anyone wants to say good job#Yeah wanting to be told good job for something as normal as losing weight is kinda sad#But I'm trying my best here#I felt the need to lie about looking for a job to them because I felt like I wasn't doing enough#Ok I'm done#Back to your regularly scheduled squid posting

so. i guess i have to go to urgent care tomorrow. & show the doctor my lab results. & try to get in for a cancer screening that way
#jesus fucking christ.#my aunt (who works in the medical field) saw my lab results bc i showed them to her#and she went 'they didnt refer you to a specialist? they didnt call you?'#and i had to explain that no they didnt bc they told me to lose weight instead#but like. i dont know how my weight correlates to my red blood cell count & other results#they referred me to a dermatologist for cancer screening because of my moles#booked that appointment and guess what. earliest was DECEMBER#b R U V#anyway if i end up getting diagnosed with leukemia i'm going back in time to 15 year old me#and telling her she is psychic. bc when i was 15 i had this gut feeling i had developed bipolar & 5 years later i got diagnoses with it#around the same time i speculated abt the bipolar i also speculated i may have some chronic form of leukemia#bc i would get these red dot patches on my arms + my doc at the time told me i had severe anemia#but they blamed the anemia on my insanely heavy periods + the lethargy on my Teen Angst™️#but i just had this sinking feeling there was more to it#& now 9 years later i feel like my body is shutting down + dying and i have lab results that prove 1) my liver is FUCKED and#2) my red blood count is FUCKED#and 3) my cholestrol is FUCKED#god if i actually do have blood cancer i'm going to scream

Advancements in Pancreatic Surgery: What Patients Need to Know
Pancreatic surgery has long been considered one of the most complex and challenging areas of surgical oncology. However, recent advancements in technology, surgical techniques, and perioperative care have significantly improved outcomes for patients undergoing pancreatic procedures. This blog post will explore these advancements used by the best pancreatic surgeons in Surat and what they mean for patients facing pancreatic surgery.
1. Minimally Invasive Techniques
One of the most significant advancements in pancreatic surgery is the increased use of minimally invasive techniques. Laparoscopic surgeries are becoming more common for certain pancreatic procedures.
Laparoscopic Surgery:
Laparoscopic pancreatic surgery involves making several small incisions rather than one large one. Through these incisions, surgeons insert a camera and specialized instruments to perform the procedure. The best laparoscopic surgeons in Surat, like Dr Dhaval Mangukiya, prefer to use this approach since it results in less pain, shorter hospital stays, and faster recovery times compared to traditional open surgery.
Benefits for Patients:
- Reduced postoperative pain
- Shorter hospital stays
- Faster return to normal activities
- Smaller scars
- Potentially lower risk of surgical site infections
It’s important to note that not all pancreatic surgeries can be performed using minimally invasive techniques. The appropriateness of these approaches depends on various factors, including the type and location of the pancreatic disease, the patient’s overall health, and the surgeon’s expertise.
2. Enhanced Recovery After Surgery (ERAS) Protocols
ERAS protocols are evidence-based care pathways designed to achieve early recovery for patients undergoing major surgery. The best gastrointestinal surgeons in Surat admit that these protocols have been successfully applied to pancreatic surgery, leading to improved outcomes and patient experiences.
Key Elements of ERAS in Pancreatic Surgery:
- Preoperative counselling and nutrition optimization
- Minimizing preoperative fasting and carbohydrate loading
- Standardized anaesthetic and analgesic regimens
- Early mobilization after surgery
- Early initiation of oral intake
- Prevention of postoperative nausea and vomiting
Benefits for Patients:
- Reduced length of hospital stay
- Decreased postoperative complications
- Improved pain management
- Faster return to normal function
- Potentially lower healthcare costs
3. Precision Medicine and Personalized Treatment
Advancements in molecular profiling and genetic testing have paved the way for more personalized approaches to pancreatic cancer treatment, including surgery.
Neoadjuvant Therapy:
For some patients, chemotherapy or radiation therapy before surgery (neoadjuvant therapy) may be recommended. This approach can help shrink tumours, making them easier to remove surgically, and may also help identify patients who are most likely to benefit from surgery.
Targeted Therapies:
Understanding the genetic makeup of pancreatic tumours allows for the use of targeted therapies. While these are primarily used in non-surgical treatments, they can complement surgical approaches in overall pancreatic cancer management.
4. Intraoperative Imaging and Navigation
Advanced imaging techniques are improving surgeons’ ability to precisely locate and remove pancreatic tumours.
Intraoperative Ultrasound:
High-resolution ultrasound probes used during surgery help surgeons identify small tumors and assess their relationship to critical blood vessels.
Fluorescence-Guided Surgery:
This technique involves injecting fluorescent dyes that accumulate in cancer cells, making them easier for surgeons to visualize and remove during the procedure.
Benefits for Patients:
- More complete tumour removal
- Reduced risk of damaging healthy tissue
- Potential for improved long-term outcomes
5. Management of Pancreatic Fistulas
Pancreatic fistulas, a leakage of pancreatic fluid, are a common and serious complication of pancreatic surgery. Dr Dhaval Mangukiya, one of the best gastro surgeons in Surat, shares that recent advancements have improved the prevention and management of this condition.
Preventive Measures:
- Improved surgical techniques for pancreatic anastomosis
- Use of somatostatin analogs to reduce pancreatic secretions
- Selective drainage strategies
Management Strategies:
- Early detection through drain fluid analysis
- Minimally invasive drainage procedures
- Endoscopic techniques for fistula closure
These advancements have led to a reduction in the incidence and severity of pancreatic fistulas, improving overall surgical outcomes.
Conclusion
The field of pancreatic surgery has seen remarkable advancements in recent years, offering new hope to patients facing pancreatic disease. Minimally invasive techniques, enhanced recovery protocols, precision medicine, advanced imaging, improved complication management, and multidisciplinary care have all contributed to better outcomes and experiences for patients.
For patients facing pancreatic surgery, staying informed about these advancements can help in having productive discussions with healthcare providers. It’s important to ask questions about the latest treatment options, the surgeon’s experience with various techniques, and what to expect before, during, and after the procedure.

its really good that I took today off because not only did I start getting treatment for the tooth pain and have the time to prep the new storage unit, I also made a bunch of health care related calls and avoided a potentially disastrous appointment time. but goddamn managing your health appointments even with relatively mild chronic issues is such a hassle if you have multiple providers
#why was I on for easter monday???? why is eastee in march this year anyway#also the oral medicine ppl asked me today if I had a neurologist for the migraines 😭 literally one of the only specialists I havent seen#I got my skin lady. my liver guy. my heart lady. my eye lady#dentist and oral med AND now orthodontist. seeing a pulmonologist for my lungs#whats next??? immunologist? podiatrist?? otolaryngologist???#well its not like im complaining. my insurance is good so of course im going to seek care if I need it. but whee#whew*#cor.txt
